Summary
Fixes three cross-review issues: Phase number mismatch (Phase 1 vs Phase 0 context check), missing incremental QA guidance in orchestrator templates, and missing post-build user handoff step.
Motivation
Same fixes as open PR #6 — Phase 5-5 said "Phase 1" while templates use Phase 0; templates regress to waterfall QA; users don't know how to trigger the harness after build.
