No default values provided when creating new user-assigned identity in Function App Identity flow

Repro Steps:

1. F1 -> Select "Azure Functions: Add Function App Identity Connections..." -> Select a function app -> Select the connections and click "OK" -> Select "+ Create new user assigned identity".
2. Observe the identity creation form.

Expect:
Provide sensible defaults:

• Identity name (e.g., -identity).
• Resource group (default to Function App RG).
• Location (default to Function App location).

Actual:

• Identity name requires manual input.
• Creating a new resource group requires manual name input.
• Location must be explicitly selected.
• No default values or suggestions are provided.
