Fix: the code review's fix prompt now carries the issues (a model without sessions saw only 'the review above'); stale spec text in the design review note. Refactor: medharness/workflows/ removed; init.py is scaffold.py, cr_state.py is services/cr_state.py; test helpers lose the underscore. Bumps to 0.54.1.
