feat(consent): send every outbound text through one consent gate - #43
Context for reading that ask: item 2 was Attend's real SMS consent record (provenance, quiet hours, working STOP) enforced in plain code before any send. The owner's rule: "Consent is a hard rule, not a judgment call: no recorded consent, outside quiet hours, or after a STOP reply means the send is blocked by plain code with no model involved." The plan is a five-PR series. PR 1 (an append-only consent ledger, ConsentEvent, and refusing sends without a grant) landed as #42. This task is the next one, plan PR 2: the single choke point every outbound text must pass. In the plan's terms, PR 2 is: a pure send gate (src/lib/send-gate.ts) and one send module (src/lib/sms-send.ts) that is the only holder of a Twilio client; the send route rewritten around them, deciding and writing in one transaction under the customer's FOR UPDATE row lock, with a permit only the gate can make; on refusal an AuditLog row (sms.send.blocked, naming the reason, user and conversation, with body length but not body) and no Message row, so the unanswered alert is not cleared; Message.consentEventId recording the grant a sent text relied on; Twilio error 21610 recorded as a REVOKED / PROVIDER_BLOCK consent event and refused; the composer showing why sending is blocked (STOP date and the START instruction, the carrier-record message for 21610); quiet hours wired into the gate but always open until a later PR; tests/send-gate.test.ts and tests/single-send-path.test.ts, where adding a Twilio import elsewhere or a forged permit cast fails the suite (demonstrated by a commit that fails and is reverted); and a ledger read that throws returns 503 and sends nothing. Keyword handling, review alerts, staff-recorded events, the real quiet-hour window, audit surfaces and docs are later PRs.
yeah same worker fix it
Context for reading that ask: two unresolved CodeRabbit threads block the merge of #43 (conversation resolution is required). The agreed fix in this PR: (1) the thread page's send-block preview in src/app/(app)/inbox/[conversationId]/page.tsx must fail soft, so a failed read shows sending as blocked rather than crashing the inbox, never falling back to allowed; (2) when the post-send write fails after Twilio already accepted the text, the advisor must not be told "Nothing reached the customer"; report that delivery is unknown instead. A durable send-attempt record that fully prevents duplicate sends is a separate follow-up, not this PR.
What Changed
Added a pure send gate (src/lib/send-gate.ts) and a single send module (src/lib/sms-send.ts), which is now the only code holding a Twilio client. /api/messages/send now just calls sendCustomerText. The gate refuses a text for NO_CONSENT, REVOKED, LAPSED (store-started texts more than 18 months after the last grant or customer text) or QUIET_HOURS (wired in, but the window stays open for now). The decision and the Message write happen in one transaction under the customer's FOR UPDATE row lock (lockCustomerConsent). A refusal writes an sms.send.blockedAuditLog row with the body length but not the body, and no Message, so the unanswered alert stays up. A failed consent read returns 503 and nothing is sent. Twilio error 21610 is recorded as a REVOKED / PROVIDER_BLOCK consent event and refused. A new nullable Message.consentEventId column (with its migration) records the grant each sent text relied on.
The composer now shows the gate's own reason for blocking a send: the date of the block, when texting opens again, and what to do next (for example, text START, or the carrier-record message for 21610). The thread page reads this with previewSendBlock. That read fails soft: a read error shows sending as blocked, never as allowed, and does not crash the inbox. When Twilio accepted a text but the write after it failed, the advisor sees that delivery is unknown instead of "Nothing reached the customer", and the draft is cleared so the text isn't sent twice. consentBlockMessage is removed from src/lib/consent.ts.
Added tests/send-gate.test.ts and tests/single-send-path.test.ts. The second fails if another module imports Twilio or casts its own permit; a commit on this branch shows it failing, and that commit is reverted. Also updated the consent PRD, the PRD index, the verify skill and AGENTS.md/CLAUDE.md to describe the send path.
⚠️ Medium: The gate, the permit, the shared row lock, refuse-without-a-Message and the 21610 handling all match the intent and are covered by database-backed tests, but two error-handling gaps on rare DB-failure paths can hide the delivery-unknown warning, or leave a never-sent QUEUED row that clears the unanswered alert.
Testing
I stood up the app as an end user runs it: an isolated Postgres 16 container, migrated and seeded; the Next dev server connecting as a non-superuser role so I could inject database faults; and Twilio replaced by a local stand-in (a CONNECT proxy in front of a TLS server with a throwaway CA, reached through twilio-node's HTTP_PROXY). I drove each scenario through a headless Chrome composer as the service advisor and the GM, plus direct POSTs to /api/messages/send.
Results:
- Refusals: no-consent and STOP sends were refused with 403. Each left an audit row and no Message row, and nothing reached Twilio.
- Happy path: a granted send went out through Twilio and was stored as SENT with the current grant's id. It was sent at 10:52 PM Chicago time, which shows quiet hours are wired in but always open.
- Carrier STOP: a Twilio 21610 recorded a REVOKED / PROVIDER_BLOCK consent event, removed the queued row, and showed the carrier-record message. The next attempt was refused before reaching Twilio.
- Generic Twilio error: returned 502, the row was marked FAILED, and managers were notified.
- Delivery unknown: Twilio accepted the text, then the stand-in revoked the app's UPDATE right on Message, so the write recording it failed. The composer showed the delivery-unknown warning and cleared the draft; the row stayed QUEUED.
- Unreadable ledger: revoking SELECT on the ConsentEvent.customerId column made the thread page render with sending blocked ("could not read this customer's consent record"), and the send API returned 503 with nothing sent.
My first attempt at the unreadable-ledger fault revoked the whole ConsentEvent table. That crashed the page inside getInboxData, which already read that table before this branch, so I narrowed the fault to the one column the gate's ledger read filters on and re-ran it.
I also ran tests/send-gate.test.ts and tests/single-send-path.test.ts against a migrated throwaway database: 44 passed, none skipped. Adding a probe file with a second Twilio client and an as SendPermit cast made the single-send-path suite fail 3 checks; it passed again once the probe was removed. Because that check is a test-suite structural guard and not a live product run, its scenario is recorded as untested.
Screenshots of every composer state are in the evidence directory. Everything I set up was torn down afterwards: the container, the servers, the browsers, .env and .next.
One thing I saw that is outside this change: while the ledger is unreadable, the thread header still shows the cached "Texting allowed" badge above a blocked composer. That is cosmetic and fails safe.

Evidence: Dev server log (local file: ~/.no-mistakes/evidence/01M65EAP5P10XFMCSW4VK8RP9V/devserver.log)
⚠️src/components/message-composer.tsx:345 - The error line now only shows when !sendBlock. That hides the DELIVERY_UNKNOWN warning in the case most likely to produce it. Here is the sequence: Twilio accepts the text, then the post-send prisma.$transaction throws on a transient DB failure (sms-send.ts:386-396). The composer clears the draft (line 225) and calls router.refresh(). If the refreshed page's previewSendBlock hits the same transient failure, it returns PREVIEW_UNREADABLE (sms-send.ts:192). sendBlock is now set, so the red "Delivery unknown... Do not send it again" line disappears. The advisor sees only "could not read this customer's consent record... Reload the page", the draft is gone, and the thread shows a plain QUEUED bubble. After a reload she has no sign the text may already have reached the customer, which defeats intent item (2). The same suppression happens if a STOP lands between the send and the refresh. Fix: track deliveryUnknown in state, or keep the error whenever the payload carried deliveryUnknown, and render it whether or not sendBlock is set. Keep hiding the plain refusal error only when sendBlock repeats it.
⚠️src/lib/sms-send.ts:407 - recordCarrierBlock runs outside any try/catch. If its transaction throws after Twilio refuses with 21610 (a DB hiccup, a lock timeout on the customer row), three things go wrong. First, the QUEUED outbound Message written before the Twilio call (line 312) is never deleted and never marked FAILED. Second, the carrier STOP is not added to the ledger, so the next send goes to Twilio again. Third, the route throws and returns an unformatted 500. That leftover row breaks the invariant this PR exists to keep ("no Message row, so the unanswered alert is not cleared"): src/lib/sla.ts counts any OUTBOUND row after the customer's last text as attended, whatever its delivery status. So a text that never went silently clears the customer's unanswered alert, and nothing marks it as undelivered. The sibling failure paths have the same unguarded post-Twilio writes: the not-connected update/notify (lines 345-361) and the Twilio-failure update/notify (lines 412-428). Those two were already unguarded in the old route, but the carrier-block path is new. Fix: wrap recordCarrierBlock in try/catch, log it, and return a refusal (403/503) that tells her nothing was sent. If that is possible, fall back to deleting the row on its own, or marking it FAILED.

⚠️src/lib/sms-send.ts:415 - This is code from fix round 1. When recordCarrierBlock's transaction throws, the whole transaction rolls back, and that includes auditBlocked(tx, target, "REVOKED", { carrierBlock: true }). The catch then only deletes the QUEUED row and returns a 403 REVOKED refusal. So a send Twilio refused with 21610 gets a refusal with no sms.send.blocked AuditLog row. The intent says "on refusal an AuditLog row (sms.send.blocked, naming the reason, user and conversation, with body length but not body)", and every other refusal path writes one. Fix: in the catch, after the fallback delete, try auditBlocked(prisma, target, "REVOKED", { carrierBlock: true, stopRecorded: false }) in its own .catch/log. The refusal response should not depend on that write succeeding.
ℹ️ tests/send-gate.test.ts:532 - This test was added in fix round 1 alongside the code it exercises. Its stub transport runs prisma.message.deleteMany(...OUTBOUND) itself, so assert.deepEqual(await outboundRows(...), []) passes because of the test's own deletion. The fallback prisma.message.delete in sms-send.ts:422 also fails in this setup, since the row is already gone. If someone removed the fallback delete, the test would still pass, so it only proves the catch keeps the 403 REVOKED refusal. Make the carrier-block transaction fail with the row still present, so the assertion actually depends on the fallback delete. Then also assert the audit-row outcome from the finding above.
ℹ️ src/lib/sms-send.ts:284 - Fix round 1 wrapped the post-send writes for 'texting not connected' and Twilio failure in recordFailedSend, and it swallows any error. If message.update to FAILED throws, the row stays QUEUED with no sid, even though the text is known not to have gone. src/lib/sla.ts counts any OUTBOUND row as attending to the customer, so that row still clears the unanswered alert, and the thread shows a pending bubble while the composer reports a failure. This leftover row predates the PR: the old route threw and left the same row. Fix round 1 handled this for the carrier-block sibling with a fallback delete, but not here. Sibling sites: sms-send.ts:369 (not connected) and sms-send.ts:438 (Twilio error). An optional follow-up: when the FAILED update throws, fall back to deleting the row.
🔧 Fix applied.

Every text now leaves through a single path that a plain-code gate
guards. src/lib/send-gate.ts decides from the consent ledger, the clock,
the recipient's zone, whether the text answers the customer or starts
something, and the contact-window policy. Nothing a model produces is an
input. A yes is a SendPermit that only decideSend can construct.
src/lib/sms-send.ts is the only module that holds a Twilio client, and
it will not send without a permit.
- The send route keeps its auth, demo, payload, length and access guards,
then hands off to sendCustomerText. That locks the customer row (the
same lock the ledger writer takes), reads the ledger and the thread,
and either writes the Message as QUEUED with consentEventId or refuses.
- A refusal returns 403 with REVOKED, NO_CONSENT, LAPSED or QUIET_HOURS.
It writes an AuditLog row, sms.send.blocked, recording the reason,
user, conversation, customer and body length (decision 10), and no
Message row, so the unanswered alert stays open. If the ledger cannot
be read, the route returns 503 and nothing is sent.
- Twilio's 21610 deletes the queued row and records REVOKED /
PROVIDER_BLOCK, dated by the database clock. The refusal is audited.
- Decision 6: a business-initiated text needs a grant or a customer text
within 18 months. Quiet hours are wired with an always-open window
(CONTACT_WINDOW), and replies already skip the window (decision 1b).
PR 4 sets the real window.
- The thread page runs the same reads as a dry run and passes the
composer sendBlock. For a STOP the composer shows the date, printed in
the browser, and how texting resumes. describeRefusal replaces
consentBlockMessage.
- Message.consentEventId (nullable, ON DELETE RESTRICT) records the grant
each text relied on.
- tests/single-send-path.test.ts fails on a second Twilio import or
client, a second caller, a SendPermit cast, or Twilio credentials read
outside env.ts. The webhook verifier now reads its token through
env.ts.
PR 2 of 5 in the consent plan.
…d path (reverted next)
This commit deliberately adds a second Twilio import and client to
src/lib/message-delivery.ts, plus a SendPermit made by a cast, to show
that tests/single-send-path.test.ts fails on both:
not ok 1 - imports the Twilio package only to send, to verify a webhook, or to sign a replay
not ok 2 - builds a Twilio client and calls its send API in one module
ok 3 - reaches the sender from the send route alone
not ok 4 - makes a permit only in the gate
ok 5 - decides from the ledger, the clock and the thread, never from a model
ok 6 - reads the Twilio credentials only in env.ts
The next commit reverts it.
The change adds a consent-aware SMS send gate and centralizes outbound sending. Outbound messages can reference the consent event used for the send. The inbox now displays send-block previews, and tests cover gate decisions, database behavior, provider outcomes, and send-path constraints.
Changes: Consent-Gated SMS Send
Layer / File(s)
Summary
Consent locking and message evidence prisma/schema.prisma, prisma/migrations/*, src/lib/consent-ledger.ts
Messages can reference the consent event used for sending. Consent-ledger operations use a shared customer-row lock helper.
Send eligibility and refusal data src/lib/send-gate.ts, src/lib/consent.ts, tests/send-gate.test.ts, tests/consent.test.ts
The gate classifies outbound messages, evaluates consent, relationship age, and contact-window rules, and returns a permit or refusal description. Tests cover these decisions and refusal wording.
The API route delegates to the send helper. The helper audits refusals, creates consent-linked queued messages, and handles provider outcomes. Tests cover database behavior and send-path constraints; documentation records the implementation and send-path rules.

A database failure after a text is accepted can tell an advisor that nothing was sent and lead them to send it again. Provider-block recording and inbox preview failures also need safe recovery before merge.

…ery unknown after Twilio accepts
The thread page's send-block preview now catches a failed read and shows
sending as blocked rather than crashing the inbox; it never falls back to
allowed. When Twilio accepted a text but the write recording it failed, the
send answers delivery unknown instead of letting the composer say nothing
reached the customer, and the composer clears the draft rather than inviting
a second send. A sent text whose alert cannot be cleared still reports sent.

Intent
do 1 and 2
Context for reading that ask: item 2 was Attend's real SMS consent record (provenance, quiet hours, working STOP) enforced in plain code before any send.
What Changed
src/lib/send-gate.ts) and a single send module (src/lib/sms-send.ts), which is now the only code holding a Twilio client./api/messages/sendnow just callssendCustomerText. The gate refuses a text forNO_CONSENT,REVOKED,LAPSED(store-started texts more than 18 months after the last grant or customer text) orQUIET_HOURS(wired in, but the window stays open for now). The decision and theMessagewrite happen in one transaction under the customer'sFOR UPDATErow lock (lockCustomerConsent). A refusal writes ansms.send.blockedAuditLogrow with the body length but not the body, and noMessage, so the unanswered alert stays up. A failed consent read returns 503 and nothing is sent. Twilio error 21610 is recorded as aREVOKED/PROVIDER_BLOCKconsent event and refused. A new nullableMessage.consentEventIdcolumn (with its migration) records the grant each sent text relied on.previewSendBlock. That read fails soft: a read error shows sending as blocked, never as allowed, and does not crash the inbox. When Twilio accepted a text but the write after it failed, the advisor sees that delivery is unknown instead of "Nothing reached the customer", and the draft is cleared so the text isn't sent twice.consentBlockMessageis removed fromsrc/lib/consent.ts.tests/send-gate.test.tsandtests/single-send-path.test.ts. The second fails if another module imports Twilio or casts its own permit; a commit on this branch shows it failing, and that commit is reverted. Also updated the consent PRD, the PRD index, the verify skill andAGENTS.md/CLAUDE.mdto describe the send path.🤖 Generated with Claude Code
Risk Assessment
Results: - Refusals: no-consent and STOP sends were refused with 403. Adding a probe file with a second Twilio client and anas SendPermitcast made the single-send-path suite fail 3 checks; it passed again once the probe was removed. That is cosmetic and fails safe.Evidence: API bypass refused for no-consent customer (403, audit row, no Message)
Evidence: Sent row records consentEventId of current grant
Evidence: 21610 recorded as REVOKED/PROVIDER_BLOCK, no Message, next send refused before Twilio
Evidence: STOP customer API bypass refused
Evidence: Generic Twilio error -> 502, FAILED row, manager alerts
Evidence: Delivery-unknown DB state and server log
Evidence: Ledger unreadable: send API 503, nothing sent
Evidence: Targeted suites send-gate + single-send-path against real Postgres
Evidence: single-send-path fails on a second Twilio client / forged permit, passes after removal
Evidence: Twilio stand-in used for the live run

src/components/message-composer.tsx:345- The error line now only shows when!sendBlock. That hides the DELIVERY_UNKNOWN warning in the case most likely to produce it. Here is the sequence: Twilio accepts the text, then the post-sendprisma.$transactionthrows on a transient DB failure (sms-send.ts:386-396). The composer clears the draft (line 225) and callsrouter.refresh(). If the refreshed page'spreviewSendBlockhits the same transient failure, it returns PREVIEW_UNREADABLE (sms-send.ts:192).sendBlockis now set, so the red "Delivery unknown... Fix: trackdeliveryUnknownin state, or keep the error whenever the payload carrieddeliveryUnknown, and render it whether or notsendBlockis set. Keep hiding the plain refusal error only whensendBlockrepeats it.src/lib/sms-send.ts:407-recordCarrierBlockruns outside any try/catch. If its transaction throws after Twilio refuses with 21610 (a DB hiccup, a lock timeout on the customer row), three things go wrong. First, the QUEUED outboundMessagewritten before the Twilio call (line 312) is never deleted and never marked FAILED. Second, the carrier STOP is not added to the ledger, so the next send goes to Twilio again. Third, the route throws and returns an unformatted 500. That leftover row breaks the invariant this PR exists to keep ("no Message row, so the unanswered alert is not cleared"):src/lib/sla.tscounts any OUTBOUND row after the customer's last text as attended, whatever its delivery status. Fix: wraprecordCarrierBlockin try/catch, log it, and return a refusal (403/503) that tells her nothing was sent. If that is possible, fall back to deleting the row on its own, or marking it FAILED.🔧 Fix applied.

If that is possible, fall back to deleting the row on its own, or marking it FAILED.src/lib/sms-send.ts:415- This is code from fix round 1. WhenrecordCarrierBlock's transaction throws, the whole transaction rolls back, and that includesauditBlocked(tx, target, "REVOKED", { carrierBlock: true }). The catch then only deletes the QUEUED row and returns a 403 REVOKED refusal. So a send Twilio refused with 21610 gets a refusal with nosms.send.blockedAuditLog row. The intent says "on refusal an AuditLog row (sms.send.blocked, naming the reason, user and conversation, with body length but not body)", and every other refusal path writes one. Fix: in the catch, after the fallback delete, tryauditBlocked(prisma, target, "REVOKED", { carrierBlock: true, stopRecorded: false })in its own.catch/log. The refusal response should not depend on that write succeeding.tests/send-gate.test.ts:532- This test was added in fix round 1 alongside the code it exercises. Its stub transport runsprisma.message.deleteMany(...OUTBOUND)itself, soassert.deepEqual(await outboundRows(...), [])passes because of the test's own deletion. The fallbackprisma.message.deletein sms-send.ts:422 also fails in this setup, since the row is already gone. If someone removed the fallback delete, the test would still pass, so it only proves the catch keeps the 403 REVOKED refusal. Make the carrier-block transaction fail with the row still present, so the assertion actually depends on the fallback delete. Then also assert the audit-row outcome from the finding above.src/lib/sms-send.ts:284- Fix round 1 wrapped the post-send writes for 'texting not connected' and Twilio failure inrecordFailedSend, and it swallows any error. Ifmessage.updateto FAILED throws, the row stays QUEUED with no sid, even though the text is known not to have gone.src/lib/sla.tscounts any OUTBOUND row as attending to the customer, so that row still clears the unanswered alert, and the thread shows a pending bubble while the composer reports a failure. This leftover row predates the PR: the old route threw and left the same row. Fix round 1 handled this for the carrier-block sibling with a fallback delete, but not here. Sibling sites: sms-send.ts:369 (not connected) and sms-send.ts:438 (Twilio error). An optional follow-up: when the FAILED update throws, fall back to deleting the row.🔧 Fix applied.
