When Ceedling stops before running any tests, every test errors with the whole transcript. The Testing view shows a message's first line, which was stdout:. Ceedling's reason, such as the ambiguous file reference of Ceedling#1364 or of a 1.2.0 coverage run of same-named files, was buried in the transcript.

Each message now leads with:

1. the most severe compiler diagnostic, when problem matching found one, as before;
2. Ceedling's EXCEPTION block: its exception line, less any decorator emoji, plus the output it carries, such as compiler errors, up to the closing could not complete operations line;
3. the transcript.

Changes

• extractCeedlingException() in ceedlingOutputParsing.ts. It reads stderr first, then stdout.
• The engine's no-report path, shared by runs and debug builds, uses it.
• New fixtures in tests/fixtures/ceedling-1.2.1/: an ambiguous reference and a compiler failure, captured with decorators on.
• CHANGELOG (Fixed), Architecture.md, KnownIssues.md, and the fixtures README.

Verification

• Test-first: 10 new unit tests failed against a stub, then passed. just test: 469 unit and 18 integration tests pass. just lint: 0 findings.
• Through the compiled engine against real Ceedling 1.2.1, on a #1364 layout, every errored test's first line is EXCEPTION: Ambiguous file reference 'test_calc.c' found. ….
