Validate and normalize allowTools at the action boundary.
addMcpBridgeUnlocked normalizes denyTools through normalizeMcpDenyTools on Line 879. It passes options.allowTools to computeToolPolicy without validation or sorting. Lines 1003-1005 say the action boundary must protect direct API callers. Those callers can skip parseMcpAddArgs, which leads to two failures:
- Invalid names reach live policy. A name that fails
VALID_ALLOW_TOOL_REis written into generated policy. The nextinspectSourceBridgeStatethen marks the entrypolicyConflict, and every later update or retry is refused. - Unsorted lists break exact retries.
requestedEntry.allowToolskeeps the caller's order, but the renderer sorts. The reconstructed entry carries the sorted list, sosameMcpAddIntentreports a definition mismatch on an exact retry.

🎯 Functional Correctness | 🟠 Major | ⚡ Quick win
Remove requireOAuth: true from this retry test, or expect a rejection.
This test expects addMcpBridge to resolve when requireOAuth: true is passed. The production path rejects that input:
addMcpBridgeUnlockedcallsenforceTransportTrust(requestedEntry, options.requireOAuth ?? false)on Line 1046 ofmcp-bridge-add-restart.ts.enforceTransportTrustthrows "--require-oauth is not supported" wheneverrequireOAuthis true (mcp-bridge-supply-chain.tsLines 59-65).
So the resolves.toBeDefined() assertion fails. The test also contradicts the PR contract that OAuth requirements are rejected. That contract is covered in mcp-bridge-input-validation.test.ts Lines 34-45 and mcp-bridge-supply-chain.test.ts Lines 10-21.
To test fail-open adoption of an identity pin, pass only serverIdentity.

Close enforceTransportTrust. The file does not parse.
The } on Line 90 closes if (transport === "stdio"), not the function. As a result, inferTransportFromUrl and computeToolPolicy are nested inside enforceTransportTrust. An export that is not at the top level is a syntax error, and Biome reports it on Lines 100-108 and 117-133. TypeScript rejects it too.
Every importer of this module then fails to build, including mcp-bridge-add-restart.ts. That means npm run build:cli fails, along with test, test:fast, and any test that imports the MCP add path.
The inner if (requireOAuth) on Lines 79-85 is also unreachable, because Lines 59-65 already throw for requireOAuth. Remove it as part of the same fix.

🔒 Security & Privacy | 🟡 Minor | ⚡ Quick win
Correct the --clear-allow-tools error text. It names the wrong security mode.
Clearing the allowlist removes deny-by-default enforcement. clearMcpBridgeAllowToolsUnlocked renders policy with no tool selectors, and the renderer then allows unparameterized tools/call. The message says the bridge returns to "deny-by-default mode", which is the opposite. The help text in mcp-bridge.ts Line 423 is correct: "return to denylist mode". An operator who reads this error can underestimate the access that the command grants.

Outcome
MCP bridge configuration now supports supply-chain hardening through server identity pinning, transport trust enforcement, deny-by-default tool allowlisting, and OAuth requirements. The change also hardens MCP registration/recovery, policy rendering, migration output, and related test infrastructure while preserving compatibility with existing policy state.
Reason
MCP servers can introduce supply-chain, transport-trust, and tool-access risks when their identity, transport, or permitted tools are not explicitly constrained. The existing bridge also had gaps in CLI usage parity, test isolation, policy migration output, and recovery handling that could allow configuration drift or make security-sensitive behavior harder to validate.
