Hook Deno standard writable streams and clean up hook module structure - #19

Refactored hook.ts to position all import statements cleanly at the top of the file, modularized hook setup into focused helper functions (hookConsole, hookNodeStreams, hookDenoSyncStreams, hookDenoWritableStreams), and added Deno.stdout.writable / Deno.stderr.writable stream hooking to prevent tasks from being duplicated when process output is streamed in Deno.
